Target Population

Clem-Mar House, Inc. is open to the general public, over the age of eighteen (unless otherwise evaluated), regardless of race, sex, religious affiliation, or sexual preference. The target population to be served is adult chemically dependent persons who reside within the Commonwealth of Pennsylvania and are a minimum of 18 years of age (unless otherwise evaluated appropriate).

Intake Criteria

Incoming referrals: Referrals will be accepted by phone, letter or mail; an attempt will be made to contact the individual within 3-5 days from receiving the referral. If there is availability the client will be provided with an intake date as soon as possible. Otherwise, the individual will be placed on a waiting list and /or will be referred to other agencies. Individuals will be prioritized according to the highest level of need.

HOURS:

The goal of the Clem-Mar House, Inc outpatient and intensive outpatient service is to broaden the current drug and alcohol treatment services to our current clients. The services will be supervised by the project director and facility director. The following is a schedule of operation:

Thursday 9:00am-5:00pm
Friday 1:00pm-8:00pm
Saturday 9:00am-5:00pm

POLICY: Clem-Mar House, Inc. is open to the male general public, over the age of eighteen (unless otherwise evaluated), regardless of race, sex, religious affiliation, or sexual preference.

Admissions Criteria:

During the intake process the counselor will evaluate the client for appropriateness of level of care required to meet the individual's needs on an outpatient basis. This process shall be based on the following:

  • Pennsylvania Client Placement Criteria (PCPC)
  • Diagnosis of Psychoactive Substance Dependence (DSM IV)
  • Must meet a minimum, PCPC level 1A or 1B criteria
  • Moderate emotional/behavioral complications
  • Supportive external environment for recovery
  • At Least a Limited understanding of recovery process
  • Willing to participate in the level of care
  • English speaking
  • Must have proper documentation of physical exam and TB test results within the past year (signed by Doctor and /or nurse)

The CWA-A Scale will be used in order to determine if the client may be suffering current withdrawal symptoms at the time of intake. This will also aid in the determination of whether the client might require a higher level of care not provided for at Clem-Mar House, Inc.

NOTE: It is anticipated that certain individuals seeking treatment at Clem-Mar House, Inc. will be better served by other community agencies or programs. Clem-Mar House, Inc. staff will assess each individual and initiate the proper referral when it is deemed more beneficial for the prospective client.

Limits to Admission

  • Clem-Mar House, Inc. will not accept clients that have open oozing lesions on their body.
  • Clem-Mar House, Inc. will not accept clients that are unmanageably combative or that may have to be restrained in any manner.
  • Clem-Mar House, Inc. will not accept clients that are severely handicapped. (special assessment for physically handicapped will be provided.)
  • Clem-Mar House, Inc will not accept clients with a criminal record for sex offenses within the past 3 year, or a criminal record for sex offenses involving children.
  • Clem-Mar House, Inc will not accept clients with a recent history of fire-setting behaviors.
  • Clem-Mar House, Inc will not accept clients who have recently experienced hallucinations (auditory or visual) or delusions, or who are on anti-psychotic medication such as: Haldol, Navane, etc. It is anticipated that these clients require a higher level of care and monitoring that Clem-Mar House, Inc. cannot provide.
    1. Admissions Procedures:

      1. Pre-arranged - When possible, referrals to Clem-Mar House, Inc. Outpatient and Intensive Outpatient Program will be pre-arranged. These referrals may come from community agencies, single county authorities, local hospitals, inpatient rehabilitation facilities, self referrals, or family members. An appointment will be made for the initial interview and evaluation at the earliest business day.
      2. Direct Phone Contacts - Requests for treatment and/or information about Clem-Mar House, Inc. may be initiated over the telephone. A pre-admission or phone contact form will be filled out by Clem-Mar House, Inc. and an interview arranged. Pre-admission/phone contact forms will be designed to gather the following information:
        Client name, address, and telephone number.
        Birth date (month/day/year).
        Referral source, including address, phone number
        Family physician.
        Insurance information or availability of resources to cover the cost of the Clem-Mar House, Inc. services.
        Client statement about personal concerns/problems.
        Previous treatment history.
        Handicaps, if any.
      3. Direct walk-in - A person presenting themselves at Clem-Mar House, Inc. reception area will be given information about the program and an initial interview/evaluation will be given. Note: Funding source information must be verified prior to admission.

      Program Outline

      Three Phase System

      Phase One - Orientation

      Time to get acquainted with the facility, staff, take time and learn to focus on "self", to identify individual needs, and to develop a stronger recovery program.

      Phase Two - Healing and Growth

      Client begins to identify core issues and personal patterns of behaviors contributing/triggering relapses and chemical dependency; addressing personal needs identified and begins working on long-term goals. Clients also must work, volunteer services, or participate in vocational training.

      Phase Three - Discharge and Continuing Care Planning

      Development of continuing care plans designed to provide a smooth transition into a life outside the program; finalize discharge plans, complete and review continuing care recovery plan of action with Primary Therapist.
